Androscoggin Valley Council of Governments (AVCOG)’s Title VI Notification is as follows:
Your Rights Under Title VI of the Civil Rights Act of 1964
"No person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance."

Members of the public who believe they have been the subject of discrimination can file a written and signed complaint up to 180 days from the date of the alleged discrimination.
Any individual, group or individuals or entity that believes it has been subjected to discrimination prohibited under Title VI may file a complaint by completing the Androscoggin Valley Council of Governments complaint form below.
Your complaint must be signed and dated.

Discrimination Complaint Form
The complaint form is available in English, Spanish, French and Somali.
Members of the public can also access the complaint forms and instructions at AVCOG’s main office located at 125 Manley Road, Auburn, ME 04210.
Hard copy forms and instructions can also be mailed upon request.
In the case where a complainant is unable or incapable of providing a written statement, a verbal complaint of discrimination may be made to the Civil Rights Officer. Under these circumstances, the complainant will be interviewed, and Civil Rights Officer will assist the complainant in converting the verbal allegations into writing.
